FOPH: Capital Income of Health Insurers for Premium Reduction – current voluntary System deemed sufficient

On 14 October 2026, the Federal Council approved its report in fulfilment of Postulate 22.3930 Feller “Use the entire capital market income of health insurers to reduce premiums”. The report examined whether health insurers should be required to apply their entire capital income towards premium reductions. The Federal Council concluded that such a requirement was not warranted.

Under the current framework, insurers may deduct a portion of their capital income from the costs to be covered by compulsory health insurance when calculating premiums (art. 25 of the Health Insurance Oversight Ordinance; Krankenversicherungsaufsichtsverordnung, KVAV). This deduction is voluntary. Most insurers apply it where their solvency is sound. In the Federal Council’s view, the existing system had proven effective and already contributed to lower premiums. Its voluntary nature avoided additional premium volatility and allowed capital income to be allocated to reserves where required. The Federal Council therefore saw no need to introduce a mandatory requirement at present.
