FOPH: Increased reimbursement for specialised palliative care under the Health Insurance Act (HIA)

The Swiss Federal Council approved an amendment to the Ordinance on Health Insurance (HIO) to improve reimbursement of specialised palliative care services, implementing a mandate arising from the Federal Council's report of 25 June 2025 in fulfilment of Motion 20.4264 ("Appropriate Financing of Palliative Care").

Under the current system, reimbursement of outpatient and nursing home palliative care is shared between compulsory health insurance, the insured person, and cantonal residual financing, a structure the Federal Council found inadequate for specialised palliative care. As a transitional measure pending the introduction of uniform financing in 2032, the FDHA has amended the Health Insurance Benefits Ordinance (HIBO) to increase compulsory health insurance contributions for specialised palliative care, effective 1 January 2027. To avoid inadvertently raising cost-sharing ceilings for all insured persons, differentiated maximum thresholds are introduced specifically for palliative care patients (CHF 22.55/day outpatient; CHF 30.40/day in specialised nursing homes). Cantons retain their residual financing obligations and may continue to reduce or waive patient cost-sharing contributions.
